If you would like to save time at the start of your appointment, you can print and fill out your intake paperwork packet before your visit. This may mean getting to sit with your preparer sooner. You will need to complete the following forms: 1. IRS Form 13614, 2. the CASH Consent Form, 3. the CASH Questionnaire, and 4. the New York State Questionnaire. You may be given additional forms or worksheets at the site, depending on how your return is being prepared and specifics of your tax situation.
